What is the Reablement Service?
The Reablement service provides short term care and support to adults age 18+, within their own homes.
The service will support individuals with activities of daily living and promote independence through therapeutic interventions.
We work with individuals to assist them to regain skills and confidence after a period of illness or injury.
Adopting a strengths-based approach, individuals will have an agreed personalised plan describing care and therapy interventions that will contribute to the achievement of individual goals, maximizing independence and well-being at every opportunity.
The care and support provided may include activities such as support with personal hygiene tasks, assisting to prepare meals and drinks, medication support, support with mobility, and focuses on:
- Individuals strengths and what they can do to stay independent and improve wellbeing
- Rebuilding confidence after a stay in hospital, illness or deterioration in health
- Restoring independence
- Preventing re-admission to hospital or a premature move to a care home
- Aims to reconnect individuals back into the community and reduce social isolation and loneliness
The reablement service is usually provided for up to 4 weeks, depending on assessment of need.
